Shaloch Manos Order Form

Thank you for participating in our Shaloch Manos Fundraiser!
Pricing for names on our list:
$250 (entire list); $125 (send to MJP staff & families); $150 (60 names); ​​$100 (35 names); $5 (per name)
     
Pricing for Personal Packages: 
$25 (package picked up at MJP);
$30 (delivered within Northern Milwaukee County or Ozaukee County; $40 (mailed anywhere in the US)​

On-line Ordering Instructions 

  • Click here to go to ordering website.  
  • Enter your login name and password which is included on the letter that was sent to you.
  • If you did not receive an order form in the mail, or do not remember your log-in information, please email us. 
  • Use the "Choose All" button at the top of the form to send to the entire list.
  • To choose individual names from the list, click on the names you want (the star will become gold & name bold). ​ To remove a name from your list, just click on the name again. NOTE: your name will not be listed on the online form​
  • To send a personal shalach manos package to names not on the list, click on <Place Order> button. Personal packages include a personalized card to the recipient.  Please be sure to let us know what you want it to say! 
  • To save your list to return to it at a later time to finish, just click <Save>
  • Once you have chosen the names you want, click <Save> at the top of the name list and then click <Place Order> button below. This will submit your order and take you to the payment page.
  • To pay by check, please send check for amount owed to: Mequon Jewish Preschool, 11112 N Crown Court, Mequon, WI 53092.
  • ​All orders and payments must be received by February 15
​All orders and payment must be received by March 6, 2019
to ensure your name is included on the cards sent with the packages.
