Also, bring in a STATE Health form & immunizations FORM SIGNED BY YOUR CHILD'S DOCTOR
(children under the age of 2 must have their form updated every 6 months, and those ages 2-5 every two years. Immunizations need to be included.)

- Household Size-Income Statement (wi.gov) (MJP Scholarship and State Eligible families must return by 1st day of attendance. Optional for other families.)
- Medical Statement Template (wi.gov) (For children needing any food substitutions. Form must be signed by a Licensed Healthcare Professional. Soy milk will be served as a milk substitute)
- Authorization to Administer Medication - Child Care Centers, DCF-F-CFS0059 (wisconsin.gov) (must have with any prescription or over-the-counter medications (including diaper rash cream)
